Insurance Information

Insurance for vision and the eyes is somewhat unusual and can be confusing to people new to the insurance business. Doctor visits relating to routine vision issues, such as nearsightedness or contact lens services, are covered by Vision Plans. Doctor visits relating to medical problems, such as red eyes, dry eyes, glaucoma or cataracts, are covered by Medical Insurance Plans. Please have current information for both when you call to schedule and when come in for your appointment so that we may bill your visits appropriately.

Please remember that both vision and medical insurance plans are a contract between you and that company. The doctor’s office has a separate contract to provide certain services. Once you use an insurance plan for your visits or eyewear, then the insurance company may be responsible for various warranties. When patients pay for the glasses themselves, then our office is responsible for any warranties and we try everything we reasonably can to get you high quality products that work.

Frequently Asked Questions

How do I know my vision insurance benefits?

This all depends on who your insurance is provided through. If it’s through work, you should have an insurance book or you can contact your human resources (HR) department. The information may be listed on a vision or medical card that was issued to you. If the vision plan is through a medical insurance company, contact them directly to see if your insurance plan was contracted out to someone else. For example, Regence may have contracted your plan out to the Vision Service Plan (VSP).

Also, be aware the some plans are not insurance plans at all. For example, Eye Med is a discount plan (not a true insurance plan) that was set up by the giant Italian frame maker, Luxottica, as a way of selling frames and funneling people into their commercial stores, such as Lenscrafters, Pearle, Sears and Vista. Discount plans are not required to follow the same rules as insurance plans in protecting patient health and rights.

It may help to contact who you have your insurance through, whether that means your place of work or your current medical provider. It is best to determine this prior to your appointment to avoid delays or problems at the actual exam.

What do I need to bring to my appointment?

Please bring current copies of your driver’s license (or another form of id), a copy of your insurance card with your insurance ID number and any current glasses or contacts that you wear. To save time, you may want to fill out our office forms before your appointment.

Directions to Our Office

From I-5 heading North: Take the 181A Exit for 44th Ave West. At the bottom of the off ramp, turn right and go up the hill. At the top of the hill is a shopping center with a Safeway (formerly Albertsons). We are in one of the suites facing 44th.

From I-5 heading South: Take the Lynnwood exit heading and turn West on 196th. Continue for about 8 blocks until you reach 44th Ave West. At that intersection, you will see the big Fred Meyer store across the way. Turn left and drive until you go under I-5 and up the hill. At the top of the hill is a shopping center with a Safeway (formerly Albertsons). We are in one of the suites facing 44th.

The Family Vision Clinic believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. To learn more about our payment options, please call our office directly.
